New clinical trial data published in July 2026 shows that inclisiran, a twice-yearly injectable cholesterol treatment, produces major reductions in low-density lipoprotein (LDL) cholesterol — the primary cardiovascular risk marker commonly called “bad cholesterol” — in patients at high risk of heart attack and stroke.
Inclisiran works through a mechanism called RNA interference (RNAi), which involves a synthetic piece of genetic material that silences a gene in liver cells responsible for producing PCSK9, a protein that reduces the liver’s ability to remove LDL from the blood. By reducing PCSK9 production, inclisiran allows the liver to clear more LDL from circulation. The drug requires only two injections per year after an initial loading dose — a significant convenience advantage over daily statin pills or monthly PCSK9 inhibitor injections.
The latest trial data, presented at a cardiology conference and reported by Medscape, showed inclisiran achieving LDL reductions in the range of 50 percent from baseline in appropriately selected patients. Importantly, the reductions were sustained at the twice-yearly dosing interval, consistent with the drug’s mechanism of action.
Inclisiran (brand name Leqvio, manufactured by Novartis) received FDA approval in December 2021 and is approved for adults with established cardiovascular disease or heterozygous familial hypercholesterolemia whose LDL cannot be adequately controlled with statins or who cannot tolerate statins.
Cardiologists have noted that the twice-yearly dosing schedule represents a genuine practical advantage for patients managing multiple chronic conditions, where daily pill burdens and adherence challenges can undermine long-term treatment success.
Separately, new clinical data on lipfendra, an oral pill-based approach to PCSK9 inhibition, is also emerging from early trials. If confirmed in larger studies, an effective oral PCSK9 inhibitor could extend cholesterol-lowering options to patients who prefer not to receive injections.
Heart disease remains the leading cause of death globally, according to the World Health Organization, with high LDL cholesterol being one of the most modifiable risk factors. For patients whose LDL is not adequately controlled by diet, exercise, and conventional statins, newer therapies like inclisiran represent an important expansion of the treatment toolkit.
