People who take part in colorectal cancer screening may have as much as a 43 per cent lower risk of dying from the disease, according to new estimates from researchers at Karolinska Institutet and Umea University in Sweden.

The analysis, published on 20 August 2026 in JAMA Network Open, drew on up to 14 years of follow-up from the Stockholm-Gotland screening programme. It tracked 376,511 participants, among whom 1,668 deaths from colorectal cancer were recorded. An invitation to screening was linked to a 26 per cent lower death risk, while active participation was associated with the full 43 per cent reduction, the authors reported in the peer-reviewed journal.

Colorectal cancer is among the most common cancers worldwide, but carries a favourable prognosis when caught early. Sweden offers universal screening to people aged 60-74 every two years: a kit arrives by post, a stool sample is returned for hidden-blood analysis, and any positive result triggers a follow-up colonoscopy. Stockholm and Gotland adopted the programme as early as 2008, giving researchers a long window to measure its effect. “We have now been able to show that the mortality is significantly lower than that among those who actually take part in the screening,” said senior author Johannes Blom, associate professor at Karolinska Institutet’s Department of Clinical Science and Education, Sodersjukhuset. The study itself appears in JAMA Network Open, where the researchers describe how they adjusted for contamination bias and nonadherence that had muddied earlier estimates.

The catch is uptake. Roughly one in three eligible people never completes the test, leaving much of the potential benefit unrealised. The gap matters because, as other screening research has shown, simply being invited is not enough; participation is what saves lives. Early detection turns a often-fatal disease into a treatable one, and the Swedish data suggest the lives saved are far from trivial.

The study was funded by the Swedish Cancer Society, the Swedish Research Council and Region Stockholm. The authors declared no conflicts of interest. For readers, the practical takeaway is blunt: when the kit arrives, use it – the evidence that it works has rarely been clearer.
