Chronic Stress Suppresses Sexual Desire Through Cortisol—Adrenal Fatigue Is Not the Explanation

Chronic stress affects sexual desire, but not because your “adrenal glands are exhausted.” That popular explanation is medically incorrect. Understanding what actually happens requires separating three different concepts that sound related but operate differently.

First concept: stress itself. When you experience stress—whether from work, relationships, or finances—your body activates the hypothalamic-pituitary-adrenal axis, or HPA axis. This is a real physiological system. The hypothalamus releases a hormone called CRH. That signals the pituitary gland to release ACTH. That hormone reaches the adrenal glands, which release cortisol. This cascade prepares your body for fight-or-flight response. It’s designed to be temporary.

Second concept: chronic stress. When stress continues without relief, HPA axis activity can shift from acute to sustained patterns. Cortisol production can remain disproportionately elevated over time. Your sympathetic nervous system—the accelerator that speeds up heart rate and directs blood to muscles—stays in overdrive. Meanwhile, your parasympathetic nervous system—the system that enables sexual arousal through blood vessel relaxation and genital blood flow—is reduced in its relative activity. This is one pathway through which stress can affect sexual desire.

Here’s where the “adrenal fatigue” myth fails: a systematic review examining decades of research found no evidence that adrenal glands become genuinely exhausted or dysfunctional from stress. The Endocrine Society states there is no scientific proof supporting adrenal fatigue as a medical condition. The glands continue producing cortisol normally. They don’t burn out or fail.

Third concept: DHEA and stress hormones. Under sustained ACTH signaling, the adrenal glands may produce proportionally more cortisol, and researchers have noted that this is associated with reduced production of dehydroepiandrosterone (DHEA) from the adrenal zona reticularis. This is sometimes described informally as the “pregnenolone steal”—a colloquial model suggesting that cortisol synthesis competes with androgen production. The actual physiological reality, according to current endocrine research, is that sustained ACTH preferentially upregulates cortisol-synthesizing enzymes while simultaneously reducing the activity of an enzyme called 17,20-lyase, which is involved in DHEA production. It’s not a physical substrate theft—it’s enzymatic diversion. The result is lower adrenal DHEA output. It’s important to note that most testosterone in adults is produced by the gonads, not the adrenal glands, though adrenal DHEA can contribute to the overall androgen pool.

Recent research confirms the connection between stress and sexual function. A 2026 review examined how both acute and chronic stress can affect testosterone and sexual function, including sexual desire and erectile function. A 2025 daily-life study found associations between higher subjective stress and lower concurrent sexual desire and arousal, though the authors explicitly cautioned that generalizability was limited given the study population.

But—and this matters—low libido has many causes. It shouldn’t automatically get attributed to stress or hormones. Depression affects sexual desire. Relationship problems affect it. Medical conditions affect it. Medications affect it. Hormonal changes at different life stages affect it. Stress is one factor among many.

The takeaway: chronic stress can affect sexual desire and function through interacting psychological, autonomic, and endocrine pathways. The evidence does not support the idea that the adrenal glands simply become exhausted. Understanding the actual pathways matters because it changes how you address the problem—often requiring stress reduction, lifestyle adjustments, and in some cases, a conversation with a healthcare provider.
