The main adult NHS autumn flu programme and the autumn Covid-19 booster campaign opened on 1 October 2026 in England, and millions of people are now eligible for free flu and Covid-19 booster jabs. The two programmes run side by side — but they do not have the same eligibility rules, and the age thresholds are different in a way that confuses a lot of people every year.

Who can get the free flu vaccine

For the 2026 to 2027 programme, a free NHS flu jab is available to:

  • Everyone aged 65 and over
  • Adults aged 18 to 64 who have certain health conditions — including asthma, chronic obstructive pulmonary disease (COPD), diabetes, chronic heart disease, or conditions that affect the immune system
  • Pregnant women, at any stage of pregnancy
  • Residents of care homes for older adults
  • Carers and frontline health and social care workers

  • Secondary-school-age children (nasal spray through schools, typically up to Year 11)
  • Close household contacts of immunocompromised people
  • People experiencing homelessness aged 16 and over

If you are aged 65 to 74 with no chronic health conditions, you qualify for a free flu jab — but you are not automatically eligible for a free NHS Covid booster this season. That is the most common point of confusion in the autumn campaign.

  • Adults aged 75 and over

Why is the Covid booster age limit 75, not 65?

For the 2026 autumn programme, the JCVI recommended a broader flu offer for people aged 65 and over and a more targeted Covid booster offer for those aged 75 and over and severely immunocompromised people.

People aged 65 to 74 are not in the current priority cohort for the free Covid booster, which is why the age threshold sits 10 years above the flu threshold.
